Focus overload ( too many masters filtering algorithms-medical,payor,hospital,legal)and time management ( impossible care in 10 minutes) and lack of self care by the health care professional (physical,emotional spiritual,boundary issues) leads to physical and moral PTSD for Doctors and nurses who cannot then truly advocate for patients. It use to be patients got excellent care and documentation was adequate for health professionals but not all other players. Now Documentation is primary and patient care is marginal . Physicians gave away the practice and leadership of medicine 30 years ago to Insurance, hmo’s, the lottery of malpractice, AMA ’s coding system,medicare. Triangular relationships are used in the behavioral sciences as STRESS models for research and doctors/nurses continue allowing these systems to be added ie Pay for Performance. After 25 year career in General Surgery, I have a cash based practice in Pain Management (prolotherapy/Laser/Manipulation/nutrition/exercise/life style) with no medicare/insurance/hmo contracts. Malpractice Ins low enough to discourage lawyers and yet pay for lawyers fees for frivolous suits. My patients and I decide how much if any treatment, if out comes justify continued care, and how much cost to the patient. Patients deal with their insurance contracts. Compensation ranges from 100% to free depending on patients financial environment. This is as medicine was in the past and is good therapy for the current PTSD of medicine/nursing. I don’t know how true this is, but I have heard that the original per pack federal tax on cigarettes instituted in the 90’s was done to support a certain amusement park in PA. The parent company that owns the park also owns a tobacco company that was sued and lost and had a huge judgement against them that they couldn’t afford to pay. The parent company threatened to pay the judgement by shutting down Hersey Park and liquidating its assets, putting a lot of people out of work and causing serious harm to the economy in that region. In order to keep the park open, and people employed, the federal govt instituted a $0.10 per pack tax and funneled the money (possibly indirectly, possibly even covertly) to the tobacco company to pay off their judgement without closing Hersey Park. Since we all know that tax money routinely goes into a big pool and is split up and doled out to support pet projects of various politicians, this doesn’t seem all that far fetched as you might think, considering they did start subsidiz
